You can find this item by defeating monsters, breaking boxes and barrels, or sometimes from treasure chests in Skull Cavern.

Defeat Monsters in Mines

Once Combat Mastery is unlocked, you can find monsters in the Mines, Quarry Mine, and Skull Cavern that have chances to drop Basilisk Paw as loot. The drop rate depends significantly on daily luck, so checking your fortune with the Fortune Teller each morning is best.

If you have equipped it, you will be immune to negative effects like Slowness from Slimes or weakened attacks from Jinxes. Bear in mind that the Basilisk Paw does not protect you against self-inflicted debuffs such as the Tipsy effect from drinking alcohol.
